Schedule an appointment with the Community Health Nurse to learn about risk factors and prevention for colon cancer as well as receive a FREE take home colon cancer screening kit.</p><p>All other Community Health Services are offered on Tuesday's at the Izaak Walton Center &amp; Reserve office. &nbsp;You must call to schedule an appointment ahead of time to ensure all the supplies needed for your appointment with the nurse are available.</p><p>Please call 630-483-5665 to schedule an appointment for your free screening or other services needed.</p>
